Camper & Parent Forms

Health History Form: Please download and fill out the 2012 Health History Form for this summer. You can either mail it to us or scan it and email it to us at the address provided on the form. We no longer require a physical, but we do have to have this form, filled out by you. We also need a copy of the front and back of your insurance card.
Parents' Guide 2012 (2 week campers and longer): This is the detailed information for the upcoming summer for all attendees except for starter campers. Please review this information carefully, as it will answer a lot of questions you might have!
Parents' Guide 2012 (Starter campers only): This is the detailed information for Starter Camp parents.
Permission Form: If you registered your child online, this has already been completed. If we registered your child for you, or if your registration was mailed in, please download and return the signed form to us in the mail. If you're not sure, just go ahead and mail the form in!
Trailblazer Letter: This is additional information for boys that are signed up for the Sioux Trailblazer hiking program. Please disregard unless you are in this group.

Parent Page: This form will help your child's counselor get to know more about your child, and what you want your child to gain from the camp experience.
Skill Sign-up: For all campers except Starter Camp campers. Please fill out this form so that we will know what skills your child would like to be a part of. Skills will be assigned through a priority system based upon when this form is received. (If your child is staying more than one session, you can add skills for an additional session when you reach the cart page.
Clothing Order Form: If you wish, you can preorder and pay for this clothing with the online form, and we will have the order filled and ready for your child on opening day. This form will remain online until July 1.
Arrival/Departure Form: Please fill out this form only if your child is arriving or departing by plane. If arriving by car, please do not fill out this form.
