- Medical Form

If you have had a physical within 12 months of camp, this can be attached to the health form and does not need to be signed by a physician. Otherwise, a physician must complete the physical and the form requires his/her signature.

- Permission Form

The Permission Form is a release of liability and required for attendance if you DO NOT sign up online. This form should be completed in full for your child to come under camp’s care.

The Parent’s Page is provided for you to share comments with your child’s counselor personally and confidentially. Please take the time to let us know about how we can tailor your child’s care to their needs and how we can make this summer unforgettable.
- Arrival and Departure Form

Use the Arrival and Departure Information Form to notify camp of your child’s travel arrangements. This form allows us to coordinate efforts for a camper getting to and from camp by airplane or car.
