The Parent Pack has all of the forms and information you will need to prepare your child for camp this summer. The pack is lengthy (12 pages or more, 1.2 MB or larger), so you will need to be prepared both with paper and ink in your printer!

- Medical Form

If you have had a physical within 12 months of camp, this can be attached to the health form and does not need to be signed by a physician. Otherwise, a physician must complete the physical and the form requires his/her signature.
- Skills Sheet

Learn more about specific skill offerings, requirements for each, and choose the skills you wish to take this summer by using this form.
- Permission Form

The Permission Form is a release of liability and required for attendance if you DO NOT sign up online. This form should be completed in full for your child to come under camp’s care.

The Parent’s Page is provided for you to share comments with your child’s counselor personally and confidentially. Please take the time to let us know about how we can tailor your child’s care to their needs to make this summer unforgettable.
- Arrival and Departure Form

Use the Arrival and Departure Information Form to notify camp of your child’s travel arrangements. This form allows us to coordinate efforts for a camper getting to and from camp by airplane or car.

Use this form to indicate your level of experience riding a horse.

Staff Forms
- Returning Staff Application

Returning staff may complete the printed version of the Returning Staff Application or you may Apply Online. You will be notified of a decision as soon as possible.
- New Staff Application

New Applicants - please read through the Employment section of our web site before downloading this application. Remember that you also have the option to Apply Online.
- Reference Form

An online Reference Form is also available. If you need to send an email link to a reference, click here Help. Once we have received an application, all four references, and an interview has been completed, a decision will be made and you will be contacted.
